N E W   P A T I E N T 
1. Where can I download the new patient forms, and how do I get them to you?
You have two options. 1. You can download and print our PDF forms from home, fill them out in advance, and bring them into our office at your next visit. ​2. You can download and digitally fill out our Word Document forms from home, and email them completed to robertmazurekmd@gmail.com before your next visit.

2. What insurance plans do you accept?
We accept most insurance plans, please call us to confirm. 
P O S T - O P   C A R E
Knee Arthroscopy
1. Is it okay to walk on my knee?
Yes, you may begin bearing as much weight as you can tolerate on your knee. Most people wean themselves off the crutches within a couple of days. 

2. When can I remove my bandage? 
You can remove the bandage two days after surgery. Once the bandage is removed you may shower and get the knee wet with soap and water. Do not submerge or soak the knee in a bathtub or Jacuzzi. After you have showered you do not need to re-bandage the knee, just leave it open to the air. 

3. Should I use ice?
Yes, icing, taking Advil or Ibuprofen, and elevation of the knee help with pain and swelling after surgery. All three of these can be used as needed to help reduce your pain and swelling. 

4. When do I have a follow up appointment with Dr. Mazurek after surgery?
You need to make an appointment to see Dr. Mazurek one week after your surgery to have your stitches removed. 

5. Can I have anything to eat or drink before surgery?
No, you cannot have anything to eat or drink after midnight the night before surgery. If you normally take any medication in the morning, you may do so with a small sip of water, but nothing more.

Shoulder Arthroscopy
1. Is it okay to use my shoulder after surgery?
Yes, you may remove the sling and begin using the shoulder as much as you can tolerate the day of your surgery. If you had a repair done to your shoulder, your use of the shoulder will be limited. Dr. Mazurek will instruct your family member or caretaker after surgery on how much you may use your shoulder if a repair is done. Otherwise, we encourage you to use it as much as you can tolerate.
